Achieving your weight loss goals will not be easy and will require lifelong commitment and a change in lifestyle. Surgical Associates has compiled a team of healthcare professionals to help give you the best opportunity for success. The program fee covers non-physician services. Payment of the program fee does not guarantee surgery; all physical and psychological criteria must be met.

Am I required to pay the fee?
Yes, all patients are required to pay the $200 program fee.
What forms of payment are accepted?
Payment can be made in the form of a debit or credit card. We accept Visa, MasterCard and Discover. You can also purchase a prepaid card from any major retailer.
What is the fee used for?
The fee is used to pay for services not covered by insurance. These services include, but are not limited to: unlimited pre- and post-operative education, pre- and post-operative phone support, and insurance assistance.
When do I have to pay the $200 fee?
The $200 non-refundable fee is required at the time you schedule your consultation appointment. We recommend all patients check to make sure Bariatric Surgery is a covered benefit under their insurance plan prior to scheduling your consultation.
Can I get a refund if I change my mind?
The $200 program fee is a non-refundable fee. Please note that most other bariatric programs charge a program fee for weight loss surgery.
