Co-pay & Co-Insurance
Surgical Associates is required by contract with your insurance company to collect co-insurance and deductible payments. Patients must pay their co-insurance or deductibles 10 days before the surgery date.
Some patients pay for their surgery out of pocket. Surgical Associates will accept credit cards for this payment. Patients who are paying privately must pay the fee at least 10 days before the surgery date.
Most insurance policies pay for medically necessary weight-loss surgery. We highly encourage you to call your insurance company and confirm that this is a covered benefit of your policy. Upon receipt of appropriate documentation and approval by the Bariatric Committee, Surgical Associates will submit your proposed surgery to your insurance company for approval. In cases where a patient has a primary and a secondary insurance carrier; we submit to both carriers.
The approval from your insurance company can take four to six weeks depending on how quickly Surgical Associates receives all the documentation required and on the efficiency of the insurance carrier once the documentation is received. If you have not heard by six weeks, please call your insurance carrier to check on the status. We can not schedule your procedure until we have written authorization from your insurance company.
